Physical and Occupational Therapy
Our trained and certified physical therapists develop a plan of plan of treatment towards the development and restoration of muscle function. This is accomplished through the establishment of tailored home exercise programs specific, muscle strengthening and gait training, education on the proper use of assistive devices at home, and assistance with pain management.
Our team of Occupational Therapists assist with helping patients attain effective performance of activities of daily living. Their focus is on upper extremity weakness, limited upper body range of motion, and decreased fine motor control.
Our team of Home Health professionals are available 24 hours a day and 7 days a week. We can help with obtaining medication, medical equipment, and medical supplies. Our services are patient-centered and our driven by the patient individual needs.


Medical Social Work
Our medical social workers address the patient’s social, emotional, financial, and environmental needs. These may include the following:
Referrals to Community Resources
Homemaker/MSS Assistance
Counseling
- Long-Term Planning
- Education on Advance Directives
- Transportation Needs
- Assistance with Alternate Living Arrangements
- Counseling with Medicare, Medi-Cal, and Other Insurance Benefits
SKILLED NURSING CARE
Our our team of registered nurses (RN) and licensed vocational nurses (LVN) provide medical care and treatment in order to aid the patient in the recovery process. Their services include, but are not limited to, the following tasks:
• Wound Care
• Diabetic Care and Management
• Ostomy Care
• Incontinent Care
• Post-Surgical Care
• Cardiac Health Management
• Medication Management
• Anti-Coagulation Management
• IV Therapy
• Specialized Care for other Chronic Conditions
